Continuation of existing treatment. Extending the validity of a prescription. Issuing an e-prescription by having the doctor select the correct medicine.
E-referral for further paid diagnostics, specialist therapy or treatment. CITO. Have an X-ray, MRI or CT scan done privately. Not on the National Health Service.
L1 available in pdf format. Can be sent to the home address provided. Health certificates, for employer, school or university.
L4 for Polish and foreign employers. Immediately sent to ZUS and the employer. Also for the care of a family member.
